An Intake Specialist will get the necessary demographic and Medicaid information needed to start the referral process. We’ll help to answer any questions, and address any concerns you have navigating care for yourself or a loved one.

INTRODUCTION CALL

Once we have the authorization to start services, a Patient Care Coordinator will confirm your information, and get all of your preferences to better understand your care needs. That information helps to match you with a state certified caregiver equipped to assist with your unique care needs.

CARING FOR YOU

Upon start of care, a caregiver will begin working with you on an agreed upon schedule to meet your care needs.  Our admin team and nursing staff will continue to be available to meet your needs and address any concerns or changes in your care.
